To register as a new patient at the practice. Please follow the registration process below.

Check You Live Within The Boundary Area

Enter your post code to check you live within the practice boundary area

If you live within the catchment area below and live in Corfe Mullen, Wimborne/Lytchett/Sturmister Marshall or Merley we will register you at Hadleigh Lodge, Corfe Mullen. If you live in Broadstone/Creekmoor or Canford Heath, then we will register you at Hadleigh House, Broadstone. If you have a preference, please advise us when we contact you by text or email. 

If you are already a patient and are about to move to a new address please let the reception team know by completing our change of details form. They can decide whether or not you are still within our catchment area. If you are not, and you would like to stay with the practice, please let us know by completing our out of area request form. All cases will be dealt with on an individual basis, by the practice manager and your named doctor. The main thing to consider when wanting to be registered with a GP practice not near to your home address is if you need a home visit, our GPs cannot provide a home visit to a patient outside of our area.

In order to register at Herne Hill Group Practice, you need to  complete our online form (below). Please ensure that you live within our catchment area before attempting to register.

After submitting this registration form, our team will let you know when you have been registered. If you do not hear from us within 3 full working days, please call us to check that your registration form has come through.

Registering at the Reception

Please attend our Practice within the opening times and we will provide you with the registration forms when you arrive.

When you register with the practice it can take a couple of months before your medical records are received from your previous doctor. You will be given a questionnaire to complete, which will give us important information about your medical history to assist us until we receive your notes from your previous GP. All the information you give us will be treated in the strictest confidence.

If you have never been registered with a practice in the UK we will not get any previous medical records.

If you suffer from heart disease, diabetes, asthma, epilepsy or any other chronic disease for which you need regular medication, we would like you to make an appointment with one of our practice nurses so that she can help you to assess your future health requirements. If you do not fall into one of these categories you are welcome to book to see one of the nurses for a well person health check.

If you have a child less than 5 years of age you will be asked for details of their immunisations and developmental checks. If you are unsure about your child’s immunisations status, please make an appointment with the health visitor.

Safer Surgeries

We are part of the Safer Surgeries Program

This initiative focuses on improving access to healthcare for all people but especially removing barriers for patients who might be refugees, asylum seekers and those that do not speak English This also includes support for those in vulnerable circumstances e.g. suffering from domestic violence, are homeless or who have no fixed address or ID etc.

NHS App Drop In

There is a clinic every Wednesday that can be booked into for all those having issues with or wanting the NHS app explained.

Just call reception for aid. 

Temporary Services

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.
